Associate Member Application

Any organization other than a private or public payer of prior authorization or specialty drug claims may apply for associate membership.

Non-payer stakeholders can apply for associate membership. The annual associate membership fees are $10,000 plus HST annually for the silver level and $15,000 plus HST annually for the gold level. The gold level is a requirement for those organizations who apply for a seat on the Framework and Accreditation Advisory Board or the Accreditation Panel. A full list of associate member benefits can be found in Section 4 of the PAFA Program Guide.

Drug access navigator associations and patient groups are eligible for a preferred associate membership fee of $2,500 plus HST annually. These organizations may also apply for a seat on the Advisory Board or Accreditation Panel.

Please attach a copy of your organization’s written support of the PAFA Program’s Guiding Principles as part of this application and any communication sent to employees. You may also include any information posted publicly on your website.

Your organization's statement supporting the PAFA Program’s Guiding Principles must:

  • Be on your organization’s letterhead, dated, and with your organization’s logo
  • Be signed by your President, COO, Vice President or Director, including their phone and email address
  • Confirm your organization’s support and list the Guiding Principles, found in Section 2 of the PAFA Program Guide
